Job Summary

A company is looking for a Pharmacist, Medication Management, Remote.

Key Responsibilities

• Leverage data to identify patients not meeting quality measures and collaborate with practices to overcome barriers
• Perform comprehensive medication reviews during transitions of care and make patient-centered medication recommendations
• Develop and deliver learning materials for clinicians, practices, and patients to address knowledge gaps related to quality measures

Required Qualifications

• Doctor of Pharmacy (PharmD) degree or Bachelor's degree in Pharmacy
• Must be licensed in the state of practice
• Graduate of an accredited School of Pharmacy
• Completion of a pharmacy residency or equivalent (3 or more years of experience in value-based care or community pharmacy practice)
• In-depth knowledge of medication adherence, medication spend, and pharmacy quality measures
