Job Summary

To deliver high-quality, preventive-focused care through telehealth, the full-time Delaware Licensed Telehealth Physician will provide routine visits to Medicare patients, create personalized care plans, and collaborate with an integrated care team, all while working remotely with no on-call duties or weekend hours.

Key responsibilities

• Provide routine telehealth visits with Medicare patients, focusing on E/M and Chronic Care Management
• Create and manage personalized care plans for ongoing conditions
• Collaborate with the integrated care team, ensuring thorough documentation and compliance with quality measures

Required qualifications

• MD or DO, Board Certified/Eligible in Family Medicine, Internal Medicine, or Geriatrics
• Active medical license; multi-state licensure is a plus
• Credentialed with major plans such as UHC, Aetna, and Humana
• Strong interest in preventive care, senior health, or chronic condition management
• Tech-savvy and comfortable practicing via telehealth
