Delaware Licensed Telehealth Physician

Job Summary

Delivering high-quality, preventive-focused care to Medicare patients, the full-time Delaware Licensed Telehealth Physician will conduct routine telehealth visits, manage personalized care plans, and collaborate with an integrated care team in a remote setting, Monday through Friday from 9 am to 6 pm.

Key responsibilities

• Provide routine telehealth visits with Medicare patients, focusing on E/M and Chronic Care Management
• Create and manage personalized care plans for ongoing conditions while ensuring thorough documentation in the EHR
• Collaborate with nurse case managers, NPs, and CHWs to engage patients in preventive care

Required qualifications

• MD or DO, Board Certified/Eligible in Family Medicine, Internal Medicine, or Geriatrics
• 1+ year of physician experience in family medicine or internal medicine
• Active medical license; multi-state licensure is a plus
• Experience in preventive care, senior health, or chronic condition management is a plus
• Comfortable practicing via telehealth and using EHR systems
