Job Summary
Acting as the primary point of contact for caregivers, families, and seniors with dementia, the full-time Dementia Care Navigator will enhance quality of life and ensure safety in the home while providing education and support around dementia care in a remote environment.
Key responsibilities
- Support caregivers and families by enhancing quality of life and ensuring safety for seniors with dementia
- Develop personalized care plans and coordinate medical care, including referrals and medication management
- Provide education on dementia symptoms and caregiving, utilizing available resources and playbooks
Required qualifications
- College degree, preferably in social work or psychology
- Comfortable with technology and performing the role virtually
- Experience building rapport with patients and families
- Able to manage difficult patient interactions with empathy and professionalism
- Fluent in English with excellent communication skills
