Dementia Care Navigator

Job Summary

Acting as the primary point of contact for caregivers, families, and seniors with dementia, the full-time Dementia Care Navigator will enhance quality of life and ensure safety in the home while providing education and support around dementia care in a remote environment.

Key responsibilities

• Support caregivers and families by enhancing quality of life and ensuring safety for seniors with dementia
• Develop personalized care plans and coordinate medical care, including referrals and medication management
• Provide education on dementia symptoms and caregiving, utilizing available resources and playbooks

Required qualifications

• College degree, preferably in social work or psychology
• Comfortable with technology and performing the role virtually
• Experience building rapport with patients and families
• Able to manage difficult patient interactions with empathy and professionalism
• Fluent in English with excellent communication skills
