Dementia Health Coach

Job Summary

To support the expanding GUIDE Program, the full-time Dementia Health Coach will provide compassionate peer support, assist families in navigating care needs, and connect them with community resources while working remotely with local availability for in-office training and visits.

Key responsibilities

• Provide empathetic coaching and emotional support to individuals with dementia and their caregivers
• Identify and coordinate access to community resources, including respite care and educational programs
• Collaborate with multidisciplinary team members to ensure comprehensive care planning and support

Required qualifications

• Bachelor's Degree preferred
• Valid Driver's License required; Health Coach Certification preferred
• Two years of experience in a social or health care setting or equivalent education preferred
• Experience in health coaching or related fields, or willingness to grow through training
• Bilingual and knowledge of local community resources preferred
