Dementia Health Coach

Job Summary

To support the expanding GUIDE Program, the full-time Dementia Health Coach will provide compassionate peer support, assist families in navigating care needs, and connect them with community resources while working remotely with some local in-office training and visits.

Key responsibilities

• Offer empathetic, patient-centered guidance to individuals with dementia and their caregivers
• Identify and coordinate access to respite care, community programs, and educational resources
• Collaborate with clinical and non-clinical team members to ensure comprehensive care planning

Required qualifications

• Bachelor's Degree preferred
• Valid Driver's License required; Health Coach Certification preferred
• Two years of experience in a social or health care setting or equivalent education preferred
• Bilingual skills and knowledge of local community resources preferred
• Proficient computer use and familiarity with medical terminology
