Diabetes Care Coordinator

Job Summary

A company is looking for a Diabetes Care Coordinator to support members in managing their diabetes through telephonic and digital engagement.

Key Responsibilities

• Perform outreach to onboard members and provide ongoing engagement support
• Educate members on the use of medical devices and related applications
• Analyze blood glucose trends and escalate care as needed

Required Qualifications

• High School diploma or GED
• 1-3 years of customer service experience in a healthcare setting preferred
• Certification or licensure appropriate to the role (e.g., CNA, LPN, EMT)
• Knowledge of diabetes care and connected medical devices preferred
• Must complete the Care Coordinator certificate within the first 90 days of employment
