Job Summary

A company is looking for a Director of Advance Care Planning.

Key Responsibilities
  • Design and lead the creation of a scalable Advance Care Planning (ACP) program
  • Develop and oversee implementation plans, workflows, and technology for ACP service delivery
  • Cultivate partnerships with healthcare providers and organizations to enhance program reach and impact
Required Qualifications
  • Master's Degree or Bachelor's Degree in social work, education, program management, or equivalent experience
  • Minimum of 7+ years of related experience and/or training
  • Experience in developing educational materials and conducting training programs
  • Ability to manage technology tools and databases relevant to program operations
  • Willingness to travel as required, approximately 10% of the time
