• Optimize operational workflows and performance metrics related to care coordination

Required Qualifications

• 5+ years of experience in managing care coordination, case management, or navigation teams in a healthcare context
• Leadership experience in Alzheimer's disease, dementia, brain health, or geriatric care
• Ability to thrive in fast-paced, dynamic organizations
• Experience with care coordination technology, EHRs, and telehealth platforms
• Strong operational mindset focusing on workflows, metrics, and systems
