Director of Community Health Operations

Job Summary

Leading the Community Health Worker function in a remote full-time capacity, the Director of Community Health Operations will manage field operations, develop CHW leaders, and ensure effective integration within multidisciplinary care teams while driving community-based interventions and operational excellence.

Key responsibilities

• Lead all Community Health Worker operations, including workforce planning, deployment, and performance management
• Establish standards for home visits, member engagement, and field documentation
• Monitor operational performance to improve member engagement, retention, and outcomes

Required qualifications

• 8+ years of healthcare operations leadership experience
• Experience leading Community Health Workers or similar community-based healthcare workforces
• Strong understanding of Medicaid populations and social determinants of health
• Demonstrated success in developing frontline leaders and improving operational performance
• Experience building and managing distributed field teams
