Director of Community Health Operations

Job Summary

Leading the operational backbone of community-based care, the full-time Director of Community Health Operations will manage the community health worker workforce and Community Supports across multiple counties, ensuring integration with clinical teams and accountability for performance reporting to health plans.

Key responsibilities

• Oversee operational performance of community health worker teams and Community Supports through direct reports
• Develop and execute geographic hiring plans aligned with care model conversion schedules and enrollment forecasts
• Ensure integration between community and clinical teams to operate as a unified care team

Required qualifications

• Eight or more years of experience leading community health, care management, or population health programs
• Proven experience managing distributed frontline teams across multiple counties or markets
• Familiarity with Medicaid programs, with a preference for CalAIM and Enhanced Care Management experience
• Demonstrated ability to integrate community-based teams into clinical care delivery
• Experience with audit and regulatory readiness in a Medicaid setting
