Director of Patient Care

Job Summary

Leading a high-performing Patient Care organization, the full-time Director of Patient Care will develop strategies for care delivery, oversee daily operations of Care Navigation teams, and ensure a high-touch patient experience while working remotely.

Key responsibilities

• Lead and develop a multidisciplinary Patient Care organization focused on key client accounts
• Establish and execute a strategic vision for patient care delivery, ensuring service excellence and scalability
• Oversee daily operations and collaborate with clinical leadership to ensure effective care transitions for patients

Required qualifications

• 10+ years of experience in patient operations, care coordination, or healthcare service delivery, with 5-8 years in leadership roles
• Proven success in leading operational teams and building partnerships with external clients
• Strong strategic and data-driven operational leadership skills
• Experience with self-funded employer benefit plans or bundled payment models is a plus
• Spanish-speaking is a plus
