Manage cases over the phone, perform assessments, and create individualized care plans
Educate patients on treatment options, coordinate referrals, and monitor treatment effectiveness
Establish relationships with healthcare providers, clients, and patients to provide support and information

Required Qualifications:

Graduate from an accredited LPN/LVN program with a current compact license
Experience in medical, surgical, and/or acute care settings
Strong understanding of case management and health program coordination
