E-Referral Specialist

Job Summary

A company is looking for an SS E-Referral Specialist who will handle electronic patient referrals in a home health setting.

Key Responsibilities

• Serve as the first line of customer service for external and internal customers
• Respond professionally to electronic referrals within specified time requirements and meet assigned KPIs
• Document patient referrals and communicate pertinent information to the care center team

Required Qualifications

• High School diploma or GED equivalent
• Experience in a timely and quality response environment impacting business results
• Home health or hospice experience preferred
• Customer-facing sales experience preferred
