ECM Care Coordination Specialist
Job Summary
Supporting the Enhanced Care Management (ECM) program, the remote ECM Care Coordination Specialist will assist members in navigating healthcare and community-based services, coordinating appointments, and maintaining communication among members, Care Managers, and providers.
Key Responsibilities
- Conduct outreach to assigned ECM members and assist with engagement in the program
- Coordinate medical, behavioral health, and community-based appointments for members
- Follow up with members regarding their care needs, appointments, and resources
Required Qualifications
- Experience in care coordination, case management, or related fields
- Familiarity with community resources and social service programs
- Ability to manage multiple assignments and follow-up responsibilities
- Experience documenting client or member contacts and service coordination activities
- Bilingual or multilingual communication skills based on the needs of the populations served
