Job Summary

A company is looking for an Engagement Coordinator to enhance patient experiences in a care navigation program.

Key Responsibilities

• Conduct outreach calls to newly enrolled patients to ensure readiness for clinical appointments
• Manage scheduling and follow-up communications for patient appointments to maintain engagement
• Document patient interactions and escalate clinical concerns as needed

Required Qualifications

• 2+ years of experience in a clinical support role (e.g., Medical Assistant, EMT, Patient Care Technician)
• Experience in patient outreach, care coordination, or telehealth support
• High school diploma or equivalent; formal clinical training or certification preferred
• Comfortable using care management platforms and digital tools
• Ability to work in a fast-paced, evolving environment
