A company in the healthcare industry is looking for an Engagement Coordinator II.

Position Responsibilities:

• Engage members in Case Management Programs through telephonic and written outreach
• Research missing member information, manage incoming inquiries, and provide care coordination support
• Focus on Annual Wellness Visits and care gap closure to impact HEDIS and STARS measures

Qualifications:

• High School Diploma or Equivalent; Associate or Bachelor's degree preferred
• 2+ years of related work experience
• Experience in the healthcare industry
• Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook)
• Experience working within EMR (Electronic Medical Record) preferred
