Enhanced Case Management Advocate

Job Summary

A company is looking for an Enhanced Case Management Advocate.

Key Responsibilities

• Identify individuals for Enhanced Case Management evaluation based on medical conditions and financial assessments
• Facilitate member engagement and assist with enrollment in relevant programs and coverage options
• Document and track time spent on each Enhanced Case Management member and assist in process improvements

Required Qualifications, Training, and Education

• Minimum Bachelor of Science Degree; BS in Social Work preferred
• 3-5 years of experience in a social services role
• Intermediate level experience with Microsoft Office applications
• Preferred certifications include LMSW, LCSW, or CCM, but not required
