Epic Healthy Planet Analyst

Job Summary

To drive population health initiatives, the remote contract Epic Healthy Planet Analyst will optimize workflows related to Social Determinants of Health (SDOH), implement AI-enabled Risk Adjustment processes, and operationalize care gap closures through data-driven dashboards and reports.

Key responsibilities

• Design and implement end-to-end SDOH workflows, ensuring actionable follow-up and closed-loop referrals
• Lead the implementation of Risk Adjustment workflows, focusing on Epic's Pre-Visit AI for seamless integration into clinician workflows
• Translate population health metrics into operational tools, configuring reports and outreach workflows to engage patients with care gaps

Required qualifications

• Active Epic Healthy Planet Certification is required
• Minimum of 3-5 years of Epic build experience, with at least 2 years dedicated to Healthy Planet
• Familiarity with Epic's Cognitive Computing platform and AI model integration in clinical workflows
• Proficiency in Reporting Workbench and Radar Dashboard configuration
• Working knowledge of Value-Based Care models, HCC risk adjustment, and SDOH reporting standards
