Job Summary
Collaborating with clinical and operational teams, the remote Systems Management Analyst - Epic Healthy Planet will support population health strategies and patient engagement workflows by designing, implementing, and optimizing value-based care tools and outreach campaigns.
Key responsibilities:
- Develop and optimize Cheers campaign workflows for patient engagement and care gap closure
- Provide subject matter expertise for patient engagement strategies and support ongoing optimization of outreach initiatives within Epic
- Analyze workflows and recommend enhancements to improve care gap closure and operational efficiency
Required qualifications:
- Epic Healthy Planet certification
- 5+ years of experience with Epic Healthy Planet or population health applications in a healthcare environment
- 5+ years of experience in workflow design, system configuration, and optimization of Epic tools
- 1+ years of experience with Epic Cheers functionality or patient outreach campaign workflows
- Demonstrated experience collaborating across clinical, operational, and technical teams
