Epic Healthy Planet Coordinator

Job Summary

A company is looking for an Epic Application Coordinator - Healthy Planet.

Key Responsibilities

• Support the design, build, configuration, and maintenance of the Epic Healthy Planet module
• Collaborate with clinical, operational, and technical teams to implement population health initiatives
• Troubleshoot system issues and provide ongoing production support

Required Qualifications

• Active Epic Healthy Planet Certification (REQUIRED), current and in good standing
• 3-5+ years of hands-on Epic build experience specific to Healthy Planet
• Proven experience supporting population health, care management, or value-based care workflows
• Experience with system configuration, upgrades, testing, and issue resolution within Epic
• Ability to translate clinical and business requirements into technical solutions
