Job Summary

To revolutionize healthcare delivery, the full-time Family Nurse Practitioner - GA, NC, SC, FL, MS Licensed will conduct virtual wellness visits, health assessments, and provide patient care remotely while collaborating with in-home clinical teams.

Key responsibilities

• Conduct thorough health risk assessments and wellness visits for patients in a virtual setting
• Collaborate with in-home team members to evaluate vital signs and coordinate patient care
• Provide comprehensive care management, including preventive care interventions and medication management

Required qualifications

• Board Certified as a Family Nurse Practitioner
• Active Family Nurse Practitioner License in GA, NC, SC, FL, or MS
• Demonstrated professionalism, integrity, and compassion in healthcare delivery
• Technologically savvy with experience using digital tools for patient care
