Job Summary

Seeking a full-time Family Nurse Practitioner - KY, MI, OH, IL Licensed, who will conduct virtual wellness visits, health assessments, and provide comprehensive care coordination remotely, while leveraging modern technology to enhance patient care.

Key responsibilities

• Conduct thorough health risk assessments and medication reviews during virtual wellness visits
• Collaborate with in-home team members to evaluate vital signs and arrange necessary patient services
• Provide comprehensive care coordination, including preventive care interventions and referrals to specialists

Required qualifications

• Board Certified as a Family Nurse Practitioner
• Active Family Nurse Practitioner license in KY, MI, OH, or IL
• Demonstrated professionalism, integrity, and compassion in healthcare delivery
• Technologically savvy with experience using digital tools for patient care and documentation
• Ability to work independently and adapt to a dynamic healthcare environment
