Job Summary

To revolutionize healthcare delivery, the full-time Family Nurse Practitioner - TX, LA, OK Licensed will conduct virtual wellness visits, health assessments, and provide comprehensive care coordination while working remotely.

Key responsibilities

• Conduct virtual adult and pediatric wellness visits, including health risk assessments and medication reviews
• Collaborate with in-home clinical team members to evaluate vital signs and arrange necessary patient care tasks
• Provide comprehensive care coordination, including preventive care interventions and referrals to specialists

Required qualifications

• Board Certified as a Family Nurse Practitioner
• Active Family Nurse Practitioner License in TX, LA, or OK
• Demonstrated professionalism, integrity, and compassion in delivering healthcare services
• Technologically savvy with experience using digital tools for charting and communication
• Ability to pass a national background check and maintain a valid clinical license
