Job Summary
Providing proactive outreach and care coordination, the full-time temporary Family Support Coordinator will support families of children in the Autism Resource Program by connecting them to services and resources while working remotely for a 12-week period.
Key responsibilities
- Conduct outreach and screening calls to assess family needs and identify barriers to care
- Facilitate referrals to clinical, community, and government resources while coordinating with care managers and providers
- Educate families on benefits and program requirements, providing administrative support to ensure compliance
Required qualifications
- Bachelor's degree in Social Work, Psychology, or a related healthcare field with three years of relevant experience, or an Associate's degree with four years of experience, or a high school diploma with five years of experience
- Strong computer skills, including proficiency in MS Outlook, MS Word, MS Excel, and data processing
- Ability to pass a comprehensive background check upon hire and throughout employment
- Self-motivated with the ability to work independently and collaboratively
- Professional demeanor and ability to manage multiple tasks in a fast-paced environment
