Job Summary

A company is looking for a Care Management Associate to manage enrollment and member engagement for a Long Term Care population.

Key Responsibilities

• Conduct initial review and outreach for new LTC enrollees and manage ongoing member engagement
• Coordinate Medicaid Redetermination and address eligibility concerns
• Ensure compliance with documentation and regulatory standards while supporting case management processes

Required Qualifications

• Fluency in English and Spanish
• Residence in Miami, Florida
• 2+ years of customer service experience with a focus on positive member outcomes
• 2+ years of proficiency with personal computers and MS Office Suite applications
• Strong organizational skills and ability to manage multiple priorities effectively
