Job Summary

To support patient outreach initiatives, the full-time Florida Certified Medical Assistant will conduct follow-up calls with recently discharged patients, review discharge instructions, and schedule appointments across multiple Primary Care offices, with the potential for remote work after 90 days.

Key responsibilities

• Conduct outbound calls to patients recently discharged from the hospital
• Review discharge instructions and perform medication reconciliation with patients
• Schedule follow-up appointments and track outreach efforts in the EMR

Required qualifications

• High school graduate or equivalent
• Successful completion of Medical Assistant training from an approved technical school or equivalent experience
• Current BLS Healthcare Provider Certification
• Certification or registration as a Medical Assistant from an accredited agency
• 2-3 years of experience as a Certified Medical Assistant
