Florida Licensed Ambulatory Care Pharmacist

Job Summary

To enhance patient care in a remote capacity, the full-time Florida Licensed Ambulatory Care Pharmacist will manage Comprehensive Medication Management (CMM) and collaborate with clinical teams to optimize chronic disease management and preventative care.

Key responsibilities

• Provide Comprehensive Medication Management (CMM) in collaboration with clinical care teams
• Develop medication management action plans to achieve member self-management goals and clinical outcomes
• Communicate with internal and external providers to align clinical interventions with medication-related goals

Required qualifications

• Completion of a PGY1 pharmacy practice residency and a PGY-2 Ambulatory Care Pharmacy Residency, or
• A minimum of 2 years' experience as a clinical pharmacist in an ambulatory care setting
• Florida Licensure as a pharmacist
