Job Summary
A company is looking for a Care Coordinator - Value-Based Operations.
Key Responsibilities
- Collaborate with a multidisciplinary care team to develop goals and plan interventions for high-risk patients
- Provide care coordination and disease management for patients with ongoing needs
- Conduct comprehensive clinical assessments and monitor patient compliance with care plans
Required Qualifications
- Graduate from an accredited School of Professional Nursing
- Current state licensure as a Registered Nurse (RN)
- Basic Life Support (BLS) Certification through the American Heart Association (AHA)
- Three to five years of nursing experience
- Preferred: Bachelor's of Science in Nursing (BSN) and specialty certification
