Job Summary

A company is looking for a Care Coordinator - Value-Based Operations.

Key Responsibilities

• Collaborate with a multidisciplinary care team to develop goals and plan interventions for high-risk patients
• Provide care coordination and disease management for patients with ongoing needs
• Conduct comprehensive clinical assessments and monitor patient compliance with care plans

Required Qualifications

• Graduate from an accredited School of Professional Nursing
• Current state licensure as a Registered Nurse (RN)
• Basic Life Support (BLS) Certification through the American Heart Association (AHA)
• Three to five years of nursing experience
• Preferred: Bachelor's of Science in Nursing (BSN) and specialty certification
