Job Summary

Overseeing daily care coordination operations for the Orlando/Central Florida market, the full-time salaried Florida Licensed Care Coordinator Lead will manage a team of Care Coordinators, mentor new staff, and serve as the primary clinical operations liaison while working remotely.

Key Responsibilities

• Supervise and mentor a team of Care Coordinators, including onboarding and performance support
• Process and route incoming referrals, lab orders, and home health orders to appropriate providers
• Triage patient calls and coordinate with external healthcare partners for effective communication

Required Qualifications

• Active RN license in the State of Florida (compact/multistate accepted)
• 3+ years of care coordination, case management, or clinical operations experience
• 1+ year of team lead or supervisory experience in a healthcare setting
• Experience with electronic health records (EHR/EMR) systems
• Strong knowledge of Medicare/Medicaid billing and home health order workflows
