Job Summary

A company is looking for a Care Management Coordinator.

Key Responsibilities

• Facilitate efficient Care Management services and monitor compliance with assigned responsibilities
• Conduct initial and continued stay reviews, ensuring timely discharges and coordination with patient financial services
• Provide daily and weekly reports on compliance and staff performance, and ensure adequate coverage for service needs

Required Qualifications

• Knowledge of financial and utilization review software, including ECIN, InterQual, and Kepro
• Two years of hospital care management experience
• Ability to maintain compliance with CMS/Medicare regulations
• Experience in conducting staff training for IT programs
• Capability to perform physical tasks such as walking, standing, and lifting up to 20 pounds
