Florida Licensed Care Manager

Job Summary

A company is looking for a Care Manager (RN) to develop and facilitate care management activities for members in Florida.

Key Responsibilities

• Evaluate member needs and recommend care plans to address barriers to accessing appropriate care
• Develop and monitor ongoing care plans while collaborating with providers and community resources
• Provide education to members and their families on healthcare options and facilitate access to necessary services

Required Qualifications

• Degree from an accredited school of nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience in nursing or care management
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required
• Must reside in Florida for the remote position
• Ability to work Monday - Friday, 8AM - 5PM EST
