Job Summary

A company is looking for a Care Manager (RN).

Key Responsibilities

• Develops and facilitates personalized care plans for members with physical health needs
• Coordinates care between members, families, and healthcare providers to ensure effective service delivery
• Monitors member progress and updates care plans as necessary to meet their goals

Required Qualifications

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience in care management
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required
• Experience in developing and implementing care plans
• Knowledge of community resources and healthcare services
