Florida Licensed Care Manager (RN)

Job Summary

A company is looking for a Care Manager (RN) to develop and facilitate complex care management activities for members with physical needs.

Key Responsibilities

• Evaluate member needs and develop personalized care plans to improve healthcare outcomes
• Coordinate with care providers and community resources to address unmet needs and facilitate access to care
• Monitor member progress and update care plans as necessary, providing education to members and their families

Required Qualifications

• Degree from an accredited school of nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience in care management
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required
• NICU experience required
• Bilingual Spanish speakers strongly preferred
