Job Summary

A company is looking for a Care Manager RN - Specialty Pharmacy.

Key Responsibilities

• Manage assigned and pended worklists
• Conduct inbound/outbound patient calls for clinical intervention
• Develop, implement, and monitor personalized care/service plans

Required Qualifications

• Active RN license (compact license required)
• 2+ years of clinical experience in acute, community, or clinical setting
• 1+ years of case management experience in managed care
• Experience in specialty pharmacy
• Preferred: Associate's or Bachelor's Degree in Nursing (BSN)
