Florida Licensed Care Manager (RN)

Job Summary

A company is looking for a Care Manager (RN).

Key Responsibilities

• Evaluate member needs and recommend care plans to address barriers to accessing appropriate care
• Develop ongoing care plans and collaborate with providers to meet members' unmet needs
• Provide education to members and their families on healthcare options and monitor progress towards care plan goals

Required Qualifications

• Active Registered Nurse (RN) state licensure or Compact (NLC) licensure
• Degree from an accredited School of Nursing or a Bachelor's degree in Nursing
• 2-4 years of related nursing experience, preferably in pediatric nursing or complex care
• Completion of corporate and shadowing/preceptor training
• Ability to manage a caseload of 1-30 members using the TrueCare documentation system
