Job Summary

A company is looking for a Care Manager (RN) to support pediatric members with chronic and complex conditions.

Key Responsibilities

• Evaluate member needs and barriers to care, recommending plans for optimal outcomes
• Develop and manage ongoing care plans while collaborating with providers and community resources
• Monitor member status and adjust care plans as necessary to meet individual goals

Required Qualifications

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience in care management
• RN - Registered Nurse - State Licensure required
• At least 2 years of pediatric experience with chronic and complex conditions
• Case management experience in a health plan, provider, or community-based setting
