Florida Licensed Care Manager RN
Job Summary
Supporting Medicaid/Medicare MMA membership, the full-time Florida Licensed Care Manager RN will develop and facilitate complex care management activities for members with physical needs, working remotely while adhering to a Monday - Friday 8am - 5pm schedule.
Key responsibilities
- Evaluates member needs and barriers to care, recommending personalized care plans for optimal outcomes
- Coordinates with providers and community resources to address unmet needs and ensures timely access to care
- Monitors member progress towards care plan goals and provides education to members and their families on healthcare options
Required qualifications
- Degree from an Accredited School of Nursing or Bachelor's degree in Nursing
- 2 - 4 years of related experience in clinical care or case management
- Active RN license in Florida or a Compact State License
- Experience with chronic conditions or diseases and managed care is preferred
- Strong time management and communication skills
