Job Summary

A company is looking for a Florida Licensed Physical Therapist (Care Navigator) to provide support for patient success in a remote environment.

Key Responsibilities

• Provide remote monitoring services to support patients in their home-exercise therapy program
• Direct and assist users in therapeutic exercise and functional training via a technology platform
• Maintain records and provide feedback to clinicians on patient progress while managing a caseload efficiently

Required Qualifications

• Active Florida PT license
• Graduated from an accredited physical therapy program
• 2+ years of experience in musculoskeletal health is a plus
• Tech-savvy with access to a newer model laptop and smartphone
• Experience in behavior change or health coaching certification is a plus
