Florida Licensed Case Manager

Job Summary

Collaborating with physicians, the part-time Florida Licensed Case Manager will manage individual patient programs in an inpatient rehabilitation setting, ensuring effective care coordination and resource navigation for patients with complex rehabilitative needs.

Key responsibilities

• Conduct comprehensive psychosocial assessments to identify patient needs and barriers to care
• Participate in interdisciplinary team meetings to develop and implement individualized care plans
• Coordinate discharge plans and connect patients to community resources and post-discharge support services

Required qualifications

• Current, unencumbered Florida license as an RN, LMSW, or LVN; Certified Case Manager preferred
• 1-3 years of healthcare and clinical experience in a hospital setting, with at least one year in medical rehabilitation
• Proficiency in Microsoft Windows environment (Outlook, Word, Excel, PowerPoint)
• Ability to effectively communicate in English, both verbally and in writing
• Strong organizational skills with the ability to manage multiple tasks under deadlines
