Job Summary

Coordinating comprehensive interdisciplinary care plans, the full-time Florida Licensed Case Manager RN will manage patient care from admission to discharge, ensuring effective communication among patients, families, and healthcare personnel while working remotely.

Key responsibilities

• Develop and implement discharge plans that meet ongoing patient needs and encourage compliance with medical advice
• Facilitate care progression and timely consultations while attending daily multidisciplinary huddles to address patient care issues
• Evaluate patient care plans regularly and collaborate with medical teams to ensure appropriate resource utilization and quality care

Required qualifications

• Bachelor's Degree in a relevant field
• Current Registered Nurse (RN) license
• Minimum of 7 years of relevant experience, including 5 years in case management or utilization review
• Knowledge of insurance reimbursement policies
• Experience in developing and facilitating patient care plans and discharge processes
