Florida Licensed Clinical Case Manager

Job Summary

To support a growing ACO team, a full-time Florida Licensed Clinical Case Manager will provide client-centered services, linking patients and their families to healthcare and community resources while ensuring coordinated access to care, all in a remote work environment.

Key responsibilities

• Conduct assessments, develop care plans, and coordinate services for patients to achieve optimal wellness
• Advocate for patients and educate them and their families about available healthcare and community resources
• Document all interactions and services accurately and in a timely manner to ensure continuity of care

Required qualifications

• Active and clear Registered Nurse license in the State of Florida
• 3+ years of clinical experience, with a preference for 1+ years in case management
• Strong computer skills, particularly in Excel, and familiarity with EMR systems, preferably Athena
• Ability to assess and interact with age-specific patient populations
• Positive, energetic, and well-organized with strong multitasking abilities
