Job Summary

A company is looking for a Clinical Pharmacist in Florida.

Key Responsibilities

Execute pharmacist-led disease state management via collaborative practice agreement
Maintain a patient panel for chronic condition disease management including diabetes, HTN, CHF, and COPD
Collaborate with providers/team to provide exceptional care to patients

Required Qualifications

Must hold an active, unrestricted pharmacist license in Florida
At least 3 years of pharmacy experience or ASHP-accredited PGY1 Residency
Experience in ambulatory care, managed care, or other clinical settings
Bachelor's degree or PharmD
Open to becoming licensed in additional states as required
