Job Summary

To support Medication Therapy Management (MTM) initiatives, the part-time Florida Licensed Clinical Pharmacist will conduct outbound patient consultations, complete Comprehensive Medication Reviews (CMRs), and ensure compliance with clinical guidelines in a remote setting.

Key responsibilities

• Conduct outbound patient calls regarding MTM services and provide detailed medication counseling
• Complete Comprehensive Medication Reviews (CMRs) while documenting findings in web-based systems
• Partner with healthcare teams to support quality initiatives related to Medicare Part D Star Measures

Required qualifications

• Active Pharmacist License in the State of Florida
• Doctor of Pharmacy (PharmD) or equivalent Pharmacy degree
• Minimum 4 years of experience as a Clinical MTM Pharmacist
• Experience conducting Medication Therapy Management (MTM) services and completing Comprehensive Medication Reviews (CMRs)
• Familiarity with Medicare Part D Star Measures and experience in high-volume outbound patient calls
