Job Summary
Supporting clinical pharmacy programs, the full-time Florida Licensed Clinical Pharmacist will conduct medication therapy reviews, perform patient outreach, and collaborate with care teams to enhance medication adherence and safety in a remote setting.
Key Responsibilities
- Conduct medication therapy reviews and targeted patient outreach under established protocols
- Review medication-related quality care gaps and support follow-up to improve patient outcomes
- Collaborate with providers and care teams to promote medication adherence, safety, and affordability
Required Qualifications
- Doctor of Pharmacy degree or other pharmacy degree from an ACPE-accredited College of Pharmacy
- Active, unrestricted pharmacist license in Florida
- Completion of accredited pharmacy training; residency or managed care experience preferred
- Basic knowledge of medication therapy management and drug utilization review
- Collaborative practice certification or related advanced credential may be required based on patient population
