Job Summary

To support a growing practice, the full-time Florida Licensed Clinical Social Worker will manage a caseload of high-risk patients, conduct psychosocial assessments, and coordinate care services in a hybrid work environment that is 90% remote and 10% in the field.

Key responsibilities

• Manage a caseload of high-risk socially complex patients and conduct face-to-face and telephonic psychosocial assessments
• Investigate psychological and social determinant barriers, designing appropriate interventions to close gaps in care
• Collaborate with a care team to develop and implement shared goals of care, ensuring patient and family understanding of treatment plans

Required qualifications

• Master's degree in social work
• Must be licensed as an LCSW or LISW
• Valid driver's license and good driving record
• Above average computer skills and ability to communicate effectively in written and verbal formats
• Experience in care management in a community-based setting or equivalent preferred
