Job Summary

To support a team of APP providers, the part-time Florida Licensed Collaborating Physician will provide necessary licensing and credentialing collaboration remotely, requiring a board-certified Family Medicine physician with Sports Medicine Fellowship training.

Key responsibilities:

• Provide licensing and credentialing support as required by Florida law
• Collaborate with APP providers to ensure compliance with medical practices
• Quickly update CAQH profiles in preparation for credentialing submissions

Required qualifications:

• MD or DO degree from an accredited medical school
• Active Florida medical license
• Board certification in Family Medicine: Sports Medicine
• Must not be credentialed with Careplus in SE Florida
• Fellowship training in Sports Medicine
