Florida Licensed Community Social Worker

Job Summary

Working remotely in a full-time capacity, the Florida Licensed Community Social Worker will engage with members to assess their social determinants of health, coordinate care, and connect them with community resources to enhance their well-being.

Key responsibilities

• Conduct telephonic and virtual assessments to evaluate members' medical, psychological, emotional, financial, and social needs
• Identify barriers to care and collaborate with care teams to develop and implement appropriate care plans
• Serve as a member advocate, providing education on benefits and connecting members to relevant community resources

Required qualifications

• Bachelor's degree in social work, sociology, psychology, or equivalent experience
• 3+ years of experience in healthcare and managed care, particularly in discharge planning and community resource navigation
• Extensive knowledge of local, statewide, and national community resources
• Demonstrated experience in social work care management
• Intermediate to advanced PC skills, with knowledge of digital engagement tools preferred
