Florida Licensed Community Social Worker

Job Summary

Working remotely in a full-time capacity, the Florida Licensed Community Social Worker will engage with members to assess their social determinants of health, coordinate care, and connect them with community resources while managing their own caseload.

Key responsibilities

• Conduct telephonic and virtual assessments to evaluate members' medical, psychological, emotional, financial, and social needs
• Identify barriers to care and coordinate community resources and services for members, including end-of-life planning and chronic illness support
• Collaborate with internal and external stakeholders to enhance member engagement and improve care management outcomes

Required qualifications

• Bachelor's degree in social work, sociology, psychology, or a related field
• 3+ years of experience in healthcare and managed care, particularly in discharge planning and community resource navigation
• Extensive knowledge of local, statewide, and national community resources
• Demonstrated expertise in social work care management
• Intermediate to advanced PC skills, with experience in digital engagement tools preferred
