Job Summary

A company is looking for a Complex Discharge Planner to join their Florida Medicaid team.

Key Responsibilities

• Develops and coordinates complex discharge plans based on members' needs assessments
• Interacts with providers and community agencies to ensure safe and effective transitions for members
• Educates members and their families on discharge plans and manages documentation in health management systems

Required Qualifications, Training, and Education

• Graduate from an Accredited School of Nursing or Bachelor's degree in Nursing
• 2 - 4 years of related experience, with 2+ years in acute care
• Clinical knowledge of discharge planning competencies preferred
• Knowledge of Medicare and Medicaid regulations preferred
• LPN - Licensed Practical Nurse - State Licensure required
